Rituximab in Membranous Nephropathy

چکیده
Membranous nephropathy (MN) is the most common cause of primary nephrotic syndrome among adults. The identification phospholipase A2 receptor (PLA2R) as target antigen in patients changed management MN dramatically, and provided a rationale for B-cell depleting agents such rituximab. efficacy rituximab inducing remission has been investigated several studies, including 3 randomized controlled trials, which complete partial proteinuria was achieved approximately two-thirds treated patients. Due to its favorable safety profile, now considered first-line treatment option MN, especially at moderate high risk deterioration kidney function. However, questions remain about how best use rituximab, optimal dosing regimen, potential need maintenance therapy, assessment long-term outcomes. In this review, we provide an overview current literature discuss both strengths limitations “the new standard.” (NS) adults worldwide with predominance Caucasian male individuals. Secondary causes like underlying malignancies, infections, or autoimmune disorders account 20% all cases.1Couser W.G. Treatment with rituximab in idiopathic membranous nephropathy
BACKGROUND Rituximab represents a valid therapeutic option to induce remission in patients with primary glomerulonephritis. Despite several studies proving its efficacy in improving outcomes in patients with membranous nephropathy (MN), its role in therapeutic protocols is not yet defined. METHODS We studied 38 patients with idiopathic MN treated with rituximab (in 13 patients as first-line t...
متن کاملRituximab therapy in idiopathic membranous nephropathy: a 2-year study.
BACKGROUND AND OBJECTIVES It was postulated that in patients with membranous nephropathy (MN), four weekly doses of Rituximab (RTX) would result in more effective B cell depletion, a higher remission rate, and maintaining the same safety profile compared with patients treated with RTX dosed at 1 g every 2 weeks. This hypothesis was supported by previous pharmacokinetic (PK) analysis showing tha...

Currently available monoclonal antibodies against the B cell surface antigen CD20 have been employed to explore whether specific inhibition of B cells may help improve the outcome of idiopathic membranous nephropathy (IMN) and may avoid the side effects of steroids and immunosuppressants. متن کاملRituximab in primary membranous nephropathy: first-line therapy, why not?
The ideal treatment of patients with primary membranous nephropathy (MN) and persistent nephrotic syndrome (NS) is still a matter of debate. This is a major issue since these patients may progress to end-stage kidney disease (ESKD) in 5-10 years. Steroids, alkylating agents, and calcineurin inhibitors have been suggested to achieve NS remission and prevent ESKD in this population. Treatment ben...
